The overall area where the practice is prepared to accept patients is shown below in GREEN .  However, within this area, there is a corridor which the practice has excluded.  This exclusion corridor is shown in RED which is the area between the River Severn and the railway line running along from Norton to Shrub Hill Station, then along Tolladine Road up to the A449.

Joining the Practice List

Please note that your registration with the practice cannot be finalised until both the registration and new patient health questionnaire forms have been completed and returned to reception together with some evidence as to your identity as outlined in the questionnaire.

You will be offered a new patient health check with one of our Healthcare Assistants.  An appointment time will be given you when you return the completed forms.  It is quite important that you attend this appointment.  If however, you subsequently find that you are unable to attend please advise the surgery so that the appointment slot can be given to someone else.

Please find both forms below to complete online or bring into surgery. Please Note: You must also attend the surgery with a form of photographic ID before you are officially registered with Severn Valley Medical Practice.

University Students

If you have already filled in a registration form please also fill in a patient health questionnaire below and print and return to surgery or email to address given below.

Temporary Residents

Please complete a temporary resident form.  This will allow you to be seen at the practice for up to 3 months. These forms are available at the surgery.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

You can also bring in your NHS medical card when you join the practice.
